Confirming Your Insurance Coverage
Before beginning counseling, we strongly encourage you to contact your insurance company to verify your behavioral health benefits.
To do so, please call the number on the back of your insurance card. Some plans may have a separate customer service line specifically for “Mental Health Benefits” or “Behavioral Health Benefits.” When you call, it’s helpful to ask the following questions:

Is my Wellbrook Wellness therapist an in-network provider for my plan?

What coverage do I have for outpatient mental health counseling, whether in-office or via Telehealth?

Do I have a deductible, coinsurance, or copayment? If so, what are the amounts?

Is there a maximum number of visits allowed per year?

Is a referral or prior authorization required for services?
